The dissertation is devoted to improving the results of diagnosis and treatment of wounded patients with migration of foreign bodies of gunshot origin (FBGO) by developing new and improving existing methods of diagnosis and removal of foreign bodies using radiation and magnetic technologies. Between May 2014 and May 2023, 90 cases of blind gunshot wounds of various locations were analysed, where there was migration of the bullet through various paths in wounded patients who received assistance at the Military Medical Clinical Centre of the Northern Region. Prospectively, the wounded were divided into two groups: the main group – 58 (64.4%) and the comparison group – 32 (35.6%) cases, which differed depending on the treatment method used. There were 89 (98.9%) shrapnel wounds and 1 (1.1%) bullet wound. All the wounded were male, with an average age of 43.2±4.1 years. Thus, the scientific novelty of the dissertation research lies in the fact that, based on a comprehensive clinical and experimental study and analysis of the causal factors of firearm injuries, the concept of migration of FBGO remodelling has been proposed for the first time, which allows for the systematisation of observation data. It consists of two stages: Stage I – wound channel, Stage II – migration path; three milestones: the entry point of the firearm wound (wound), the place of migration onset, the place of fixation (location of the ST) and the direction of migration. The completed dissertation work made it possible to assess the advantages and disadvantages of existing methods, to improve them, to develop new methods and techniques for the diagnosis and removal of foreign bodies during the migration of migration of FBGO. For the first time in Ukraine, it was shown that the migration of FBGO is a rare phenomenon that has its own characteristics. The concept of migration of a foreign body of firearm origin and pseudo-migration is defined. It has been proven that the migration of FBGO is a separate type of the course of a gunshot wound. For the first time in Ukraine, on the basis of mathematical and clinical studies, the characteristics of migrating FBGO were established, detailing their frequency, localization, structure and properties. For the first time, the classification of the migration of FBGO was proposed according to the following characteristics: the place of the entrance hole, the place of the beginning of migration, the direction of migration, the number of foreign bodies, the structure of foreign bodies, the distance of migration, by process, time, cause, frequency, fixation, by the place of fixation (location) foreign bodies, the volume of surgical treatment referral. It has been proven that the migration of FBGO is always limited by the boundaries of the cavity, the length and diameter of natural or pathological pathways. The main directions of migration of FBGO are defined: respiratory tract, gastrointestinal tract, vascular bed, freely located foreign bodies in cavities, genitourinary system and during suppuration in soft tissues, when the foreign body moves with the flow of pus. It has been proven that in the case of blind penetrating gunshot wounds with localization of foreign bodies in cavities where migration is possible (thoracic, abdominal, large joints), preference should be given to minimally invasive endoscopic methods of removal using modern magnetic instruments. On the basis of the performed mathematical modeling of the migration of a foreign body of gunshot origin, the probability of removal of the foreign body, its localization during further migration, which affects the scope of operative treatment, is determined.
