The dissertation is devoted to the theoretical and organizational bases of Good Pharmaceutical Practice (GPP) in the implementation of the diabetes prevention and management strategy, and theoretical positions and recommendations for their implementation in the practice of pharmacies have developed. As a result of the analytical review of the literature, the data on modern trends in the prevalence and consequences of diabetes mellitus (DM) are summarised. The significant prevalence of diabetes mellitus and unsatisfactory treatment outcomes of patients with diabetes (PWD) in Ukraine (12.7 % of patients achieve compensation of the disease) necessitate the search for solutions to this problem. A promising approach is the involvement of pharmaceutical specialists and the implementation of GPP in Ukraine for the realization of the diabetes prevention and management strategy. Using the synthesis method based on the analysis of international documents, a flowchart of the main directions of pharmacy activity in providing pharmaceutical care (PC), according to GPP at the levels of prevention defined by WHO, regarding DM and its complications is proposed. A programme-targeted structure for the preparation and execution of the study was developed. The study of regulatory documents and standards for the treatment of DM showed that the current legislation of Ukraine allows doctors use both domestic and foreign clinical guidelines for the treatment of DM to implement modern pharmacotherapy schemes. 9 A marketing analysis of the range of glucose-lowering drugs (GLDs) of synthetic and herbal origin showed that a sufficient range of GLDs is registered in Ukraine, which allows the use of modern therapy schemes for type 2 diabetes (T2D). The study of the pharmaceutical market for glucometers has proven the availability of a wide selection of glucometer models, which provides the ability to meet patient needs. Over the last decade, there has been development in the glucometer segment both quantitatively (the number of models has increased by 1.8 times) and qualitatively (expansion of device functionality). The results of the conducted study identified problematic issues in the pharmaceutical provision for PWD, including under martial law conditions, and proposed ways to improve it. Surveys revealed numerous problems with the use of medications and low adherence to treatment among patients with DM in Ukraine. A set of problems related to providing PC to PWD and the introduction of new pharmaceutical services (PS) has been identified. Preferences of PWD regarding the practice of pharmacy (availability of a wide range of products, loyalty program for diabetic products) have been determined. The implementation of additional PS in pharmacies (testing blood glucose, cholesterol, blood pressure; checking weight; calculating body mass index and interpreting results) and pharmaceutical consultations (medication intake, interactions with medications and food products, impact of comorbid conditions of the patient) have been justified. According to the opinion of PWD, the organization of a "Diabetes Self-Management School" based in pharmacies and the conduct of systematic awareness events in pharmacies for the World Diabetes Day. A comprehensive survey of doctors identified the reasons for the limited prescription of modern GLDs for the therapy of T2D there are: high price, absence in the reimbursement program, unavailability in pharmacies, and insufficient awareness of the doctor about certain groups of medications (α-glucosidase inhibitors, thiazolidinediones, DPP-4 inhibitors, repaglinides). Considering the identified factors affecting the doctor's prescription, the socio-economic accessibility of GLDs in Ukraine was studied, taking into account the reimbursement system. The analysis of the list of reimbursed drugs showed that out of the registered 161 GLDs, which contain 16 international nonproprietary names and 8 of their combinations, three names are subject to reimbursement: 10 metformin (40), glibenclamide (3), gliclazide (8), most of which (76.9%) are of domestic production. Calculations of the co-payment for Defined Daily Dose (DDD) of drugs showed that for gliclazide it ranges from 0.111 to 2.510 UAH, and for metformin from 0.105 to 1.576 UAH. Glibenclamide is fully reimbursed. The co-payment for GLDs of foreign production is higher than that of domestic production.
