Essay

Study Aim: To improve the effectiveness of medical care for children with abdominal pathology undergoing lower abdominal surgery by enhancing the diagnostic and therapeutic approaches through an in-depth investigation of the mechanisms underlying acute and chronic pain and evaluating regional anesthesia effectiveness. Study tasks: 8. To assess the frequency, clinical, and paraclinical characteristics of acute and chronic postsurgical pain in children with surgical pathology who underwent lower abdominal surgery under general anesthesia with opioid analgesia. 9. To investigate the role of the ADRB2 molecular structure polymorphism as a prognostic marker for the intensity of acute and chronic pain in children with abdominal surgical pathology. 10. To study the impact of regional anesthesia on serum levels of transmembrane proteins CD40-CD40L and pro-inflammatory mediator Toll-like receptor 4 (TLR-4) as markers of acute and chronic pain in children undergoing lower abdominal surgery. 11. To investigate the effect of regional anesthesia on local acute and chronic hyperalgesia in children with abdominal surgical pathology. 12. To study the impact of regional analgesia on the indicators of intra-abdominal hypertension, including intra-abdominal pressure and abdominal perfusion pressure, and regional hemodynamics in the superior mesenteric and renal arteries in children who underwent lower abdominal surgery. 13. To develop a mathematical model for predicting changes in serum levels of CD40-CD40L and acute pain scores under various anesthetic techniques in children. 14. To study the quality of life (QoL) of children, their parents, and families using the Pediatric Quality of Life Inventory 3.0 (PedsQL™) questionnaire, depending on the method of perioperative analgesia. Object of the study - the perioperative period in children who underwent lower abdominal surgery. Subject of the study - clinical indicators of vital functions, serum biochemistry profile, immunological markers (Toll-4, CD40-CD40L), genetic markers (polymorphism of the ADRB2 molecular structure), local hyperalgesia indicators, US and Doppler US findings, summary scores from acute and chronic pain assessment scales, and QoL assessments using questionnaires.
