The thesis is devoted to solving an actual and socially significant scientific and applied problem of improving the methodology of public administration in the domestic healthcare and pharmacy system in the context of the implementation of HTA. The use of HTA as a transparent decision-making tool at various levels of healthcare management will contribute to improving the quality of medical and pharmaceutical care (MPhC), ensuring effective accessibility and rational use of medicines for the population of Ukraine. Based on comprehensive theoretical and applied research, the methodology has been developed and the conceptual and categorical apparatus of the theory of state management and public administration has been refined. The global experience of state (public) administration has been analyzed from a historical perspective, and contemporary models and trends in the development of the healthcare and pharmacy management system have been identified in the context of reforming the government service and healthcare system towards the implementation of the principles of good governance (publicity, transparency, participation, accountability, responsibility, etc.) and ensuring the achievement of the Sustainable Development Goals. The key role of HTA in the processes of health policy formation and evidence-based decision-making on the feasibility of using and financing MT (in particular, during the procedures for approving medicines, determining price levels and reimbursement, inclusion in reimbursement lists, procurement nomenclatures, etc.) is substantiated. Based on the results of a comprehensive expert survey, an assessment of the main problems of public administration in the healthcare and pharmacy system was carried out, with particular attention paid to the analysis of the current state, problems, and prospects for the implementation of HTA, as well as the functioning of the public procurement system for medicines. The state and problems of self-government development in domestic healthcare and pharmacy were studied, and the role of public professional and patient organizations in the formation and implementation of patient-oriented healthcare policy was determined. An analysis of the institutional capacity of the domestic healthcare and pharmacy system was conducted, along with a structural and functional analysis of government agencies and regulatory documents that define policies aimed at achieving a balance between private and public interests in the healthcare sector, as well as strategic directions for reforming the industry to ensure the accessibility, quality, and safety of medicines. The state of pharmaceutical provision for certain categories of patients, in particular those suffering from rare (orphan) diseases, was investigated. Approaches to the organization of hospital-based HTA of medicines used in the treatment of orphan diseases have been improved using new indicators that are socially oriented and adapted to the economic development characteristics of national healthcare systems. Approaches to improving public management mechanisms in the field of medicines accessibility (pricing and reimbursement, public procurement, MEA, hospital HTA, improvement of regulatory lists of medicines, horizon scanning, etc.) have been substantiated. The peculiarities of the organization of HTA management systems, their role and place in the institutional structure of the healthcare system have been studied, and a typological analysis of HTA management systems has been conducted according to key characteristics: level of centralization, sources of funding, degree of implementation in the decision-making process, areas, directions, and models of HTA, and publicity. The international experience of building human resource capacity in the HTA system and its professional assessment has been analyzed and systematized. A professional standard for HTA experts has been developed, and the main competencies, learning outcomes, and job functions have been substantiated. A conceptual model of public health management, including pharmacy, has been substantiated in the context of HTA implementation with the aim of making socially oriented management decisions in the process of organizing the provision of affordable MPhC to the country's population
