The dissertation is devoted to a comprehensive study of the clinical and psychopathological structure and phenomenology of negative symptoms (NS) in schizophrenia, the development of primary and secondary negative symptoms, features of the motivational sphere, cognitive symptoms, social interaction and social functioning in such patients, determinants and mechanisms of the formation of negative symptoms, which were studied at different stages of the development of the pathological process (primary episode, exacerbation, remission), which became the basis for the development of diagnostic criteria and a comprehensive program of psychological rehabilitation of patients. The study was based on the examination of 331 patients diagnosed with schizophrenia (252 patients with a predominant clinical picture of NS, 79 patients with a predominant positive symptoms). Among patients with NS, 83 were included in the first episode group, 88 patients in the exacerbation group, and 81 patients in the remission group. To achieve the goals and objectives of the study, a comprehensive approach was used in the work, using clinical-psychopathological, clinical-anamnestic, psychodiagnostic, psychometric and mathematical-statistical methods. The dissertation work presents for the first time a comprehensive analysis of NS in schizophrenia, determines its structure, mechanisms of formation and dynamics at different stages of the disease. A structural-dynamic typology of NS in schizophrenia is determined, which includes three stably reproducible clinical-psychopathological models: motivational-deficient, emotional-apathetic and cognitive-disintegrative types. The features of the motivational sphere, cognitive symptoms, social interaction, psychosocial adaptation and social functioning in patients with NS in schizophrenia are analyzed, and typical characteristics of these features at different stages of the development of schizophrenia are highlighted. The determinants and mechanisms of the formation of NS in patients with schizophrenia were identified, and the clinical and anamnestic, clinical and psychopathological and psychological diagnostic criteria for NS in schizophrenia were identified. Based on a comprehensive analysis of the study results, comprehensive psychosocial rehabilitation program (CPRP) was developed, proven to be effective and implemented in healthcare practice. It is based on the principles of the biopsychosocial approach and is aimed at reducing the clinical signs of NS in schizophrenia, improving social and behavioral compliance and restoring social functioning of patients. The results of the testing of the developed CPRP for patients with NS in schizophrenia reflect its higher effectiveness compared to traditional psychosocial rehabilitation schemes for such patients, which do not take into account the clinical and psychopathological structure and typology of NS in this contingent of patients.
