The dissertation is devoted to improving the effectiveness of treatment of patients with generalized periodontitis ІІ-ІІІ degrees experimental and clinical substantiation of the choice of the most pathogenetically adequate complex of medical support based on the identified mechanisms of wound healing depending on the state of the body's reactivity. Experimental studies were performed on 24 dogs with spotty periodontitis. The animals were divided into three equal groups: normal body reactivity, hyper- and hyporeactivity of the body was modeled. After modeling the body's reactivity, all animals underwent periodontal surgery. As experimental animals for the morphological study, 90 rats with spontaneous periodontitis were selected. The clinic examined 780 patients with generalized periodontitis. For a detailed analysis, 216 people aged 25 to 55 were selected. The diagnosis was generalized periodontitis II, III severityhronic course. All patients were indicated for surgical intervention. Depending on the state of the body's reactivity, patients were divided into three groups: normoreaction (61%), hyperreaction (21%) and hyporeaction (18%). Patients with hyper- and hyporeactivity of the body were the main groups (they were treated with the proposed medical correction). Patients in the comparison and in the control groups received comprehensive treatment of generalized periodontitis. The dynamics of clinical and laboratory parameters were initially assessed at 1-st, 2-nd, 4-th, 6-th, and 9-th days following surgical treatment. Long-term outcomes were analyzed based on clinical and radiological data at 6 months, 1 year, and 2 years. It has been experimentally established that the body’s normoreactivity is characterized by balanced dynamics of regulatory system indicators, which leads to the synchronization of markers of necrotic and reparative processes in the blood. This ensures adequate changes in the phases of cellular reactions in the gingival mucosa and alveolar bone osteoreparation. In simulated hyper- and hyporeactivity of the body with imbalanced changes in the indicators of regulatory systems, which exert their effect through the desynchronization of the dynamics of markers of necrotic and reparative processes in the blood, inadequate changes in the phases of cellular reactions of the gingival mucosa and alveolar bone osteoreparation are identified. The clinic found that the dynamics of regulatory system indicators in patients with normal immune response were balanced, ensuring uncomplicated wound healing by the 9th day after surgery. In cases of hyper- and hyporeactivity of the body, an imbalance in the indicators of regulatory systems in the blood is observed, leading to complicated wound healing characterized by desynchronization of cellular reactions in the gingival mucosa, osteoreparation of the alveolar ridge bone tissue, and clinical healing delayed by 3–4 days. The distinctive features of the dynamics of regulatory system indicators and uncomplicated versus complicated wound healing after surgical intervention, depending on the body’s reactivity, justify the feasibility of differential postoperative pharmacological management, aimed at correcting the body’s reactivity indicators by bringing them closer in level and time to the values observed in normal reactivity, which is considered a prerequisite for optimizing periodontal tissue healing processes. An assessment of the effectiveness of the proposed differential pharmacological management in patients with hyper- and hyporeactivity in the immediate term showed that the dynamics of regulatory system indicators and the timing of clinical wound healing approached those observed in cases of normal body reactivity. The results of cytological studies indicate the synchronization of the phases of cellular reactions with the appearance of signs of active regeneration of the gingival mucosa on the 6th day, as in normoreactivity.In the long term (after 2 years), in patients with generalized periodontitis with normal body reactivity, stabilization of the process was recorded in 82.9% of cases. In patients of the main groups, the percentage of stabilization of the inflammatory and destructive process in periodontal tissues was commensurate with that in the comparison group and reached 78.3 and 78.9% of cases, respectively. In the control group, stabilization was recorded, on average, 10% less often.
