The dissertation is devoted to solving a relevant scientific and practical problem – improving the organisation of cardiological care delivery and enhancing the cardiological health of the population through scientific substantiation and development of a conceptual functional and organisational model of a regional system of cardiological care for the adult population. The relevance of the work is determined by the high prevalence and adverse consequences of cardiovascular diseases in Ukraine, as well as the need to create an effective, integrated, patient-centred, and digitally transformed healthcare system. The basis for substantiating the improvement of the regional system of cardiological care for the adult population was formed by the results of the author’s own research, priority directions for the development of the national healthcare system, and recommendations of international healthcare organisations. The information base of the author’s research included a comprehensive comparative retrospective analysis of incidence, prevalence, disability, and mortality due to cardiovascular diseases among the adult population of Kyiv region; assessment of human resources, material and technical support, and organisation of cardiological care delivery in the region; analysis of patient opinions in healthcare institutions regarding accessibility and quality of cardiological care, professional opinions of general practitioners, cardiologists, and cardiac surgeons, as well as healthcare experts concerning the current state, problems, and prospects for improving the organisation of cardiological care for the adult population. Negative trends in the cardiological health of the regional population, as well as problems in the organisation and resource provision of medical care determining low accessibility and quality of services, were identified. A steady increase during 2010-2017 in the incidence and prevalence of cardiovascular diseases among the adult population of the region by 5.7% and 8.5%, respectively, was established, along with higher regional indicators compared to national averages, and higher incidence of cardiovascular diseases among the rural population compared to the urban population by 1.2-1.5 times, which may indicate differences in access to specialised care. A trend towards increasing mortality from cardiovascular diseases was identified during 2010-2017 by 6.6% and during 2018-2021 by 9.8%. The assessment of the organisation and resource provision of healthcare delivery to the regional population with cardiovascular diseases revealed a trend towards an increase in the availability of cardiac surgeons by 28.7%, a 2.6-fold rise in the number of cardiac surgery beds, and an expansion of high-tech cardiological and cardiac surgical care. However, a reduction in the number of cardiology beds by 39.8%, a 1.7-fold decrease in their availability to the population, a 1.9-fold decline in hospitalisation rates, and reduced coverage of the population with echocardiographic and angiographic examinations (by 1.9 times and 22.4%, respectively) indicate a deterioration in the accessibility of medical care. The relatively low quality of care for patients with cardiovascular diseases is confirmed by a tenfold increase in in-hospital mortality and low patient satisfaction with the quality of follow-up by cardiologists (only 32.9%±1.9% of respondents were fully satisfied).
