Essay

The dissertation research is devoted to substantiating and developing an integrated approach to public administration in the development of family medicine in rural areas of Ukraine. The main conceptual and practical provisions have been brought to the level of applied models and practical recommendations that can be used in the framework of further improving the mechanisms of public management of the development of family medicine in rural areas of Ukraine on the basis of an integrated approach, which is especially relevant in the conditions of the period of martial law, as well as the post-war reconstruction of Ukraine. The relevance of the topic is determined by the need to overcome systemic problems in ensuring the accessibility and quality of medical services for the rural population, which have intensified due to demographic aging, migration processes, and the consequences of martial law. In accordance with WHO, EU, and national health strategies, the integration of medical, social, and managerial resources is a key prerequisite for building a sustainable model of primary health care based on family medicine. The war caused significant destruction of the medical infrastructure: according to WHO, as of April 2024, 1,682 attacks on healthcare facilities had been recorded, resulting in the deaths of 128 medical workers. These circumstances further complicated the provision of medical services in rural regions, where there were problems with infrastructure and staffing even before the war. The research applied systemic, structural-functional, comparative, legal, statistical, sociological, and expert methods, enabling a combination of theoretical analysis with a practical assessment of the effectiveness of management decisions. The purpose of the dissertation research is to theoretically substantiate and develop scientific and practical principles of public management of the development of family medicine in rural areas of Ukraine based on an integrated approach. To achieve the stated goal of the dissertation research, the following scientific tasks were formulated: 1. To analyze the evolution, current state and global trends in the development of family medicine, particularly in the context of rural areas, and to outline its significance as an institution of primary health care. 2. To summarize the theoretical and methodological foundations of an integrated approach in the field of health care and to determine its applicability from the standpoint of implementing public management influence at the level of rural communities. 3. To assess the current state, problems and barriers of the functioning of the family medicine system in rural areas of Ukraine in the context of decentralization, war and crisis sectoral transformations. 4. To identify key management deficits and potentials for the implementation of an integrated model of primary health care organization in rural areas, taking into account inter-sectoral interaction, staffing, digital tools and logistical accessibility. 5. To develop conceptual approaches and practical recommendations for improving public management of the development of family medicine in rural areas on the basis of an integrated approach under the legal regime of martial law and the period of post-war reconstruction of Ukraine. The object of the study is public relations in the field of public management and administration of the health care system. The subject of the study is public management aspects of the development of family medicine in rural areas of Ukraine based on an integrated approach.
