Vezhnovets Y. Medical and social substantiation of the improvement of the organisation of medical care for children with respiratory diseases

The dissertation is dedicated to solving a pressing scientific and practical problem – substantiation of a model for improving the organization of medical care for children with respiratory diseases (RD) to improve the quality of this care based on the prediction of morbidity, determination of the features of the provision of human resources and bed capacity of health care institutions, analysis of the clinical route of patients, research of the leading ways of parents seeking medical care for their children and their satisfaction with the quality of medical care. For the first time in Ukraine, a medical and social justification of a model for improving the organization of medical care for children with respiratory diseases was carried out, the innovative elements of which were: 1) educational and educational work with parents on providing home medical care to a child when acute symptoms of a respiratory disease develop, timeliness of seeking medical care and avoiding self-medication, fostering loyalty to communications with doctors of the ENT department and pediatricians in order to reduce unfounded cases of appeals for EMR, self-referrals to pediatricians of the hospital's admission department and the provision of medical care in conditions appropriate to the medical needs of a child with ENT; 2) improving clinical routes of patients depending on the clinical condition of the child, impeccable compliance by GPs and pediatricians with industry clinical protocols for medical care for children with CKD, streamlining hospitalization by e-referral of a pediatrician, GPs, monitoring clinical indicators of the quality of medical care for children with CKD in accordance with WHO recommendations; 3) rational planning and use of bed capacity by standardizing the number of beds per 1 full-time position of a pediatrician in a hospital, providing pediatric institutions with infrastructure components and modern medical equipment, and increasing the financial accessibility of medical care for children with CKD. Approaches to the development and implementation of clinical routes of the patient depending on the severity of the child's CKD and the healthcare institution to which the parents turned for medical care have been improved.
