At present, dental implantation is a widely used and effective method for the treatment of patients with dentition defects; however, the surgical stage of restoring dentition defects is associated with a number of complications that may lead to dental implant rejection. The aim of this study was to improve the effectiveness of comprehensive treatment in patients undergoing dental implantation in the presence of hepatobiliary dysfunction by using hepatoprotective and osteoprotective agents as pharmacological support in order to reduce the incidence of complications. To achieve this goal and obtain scientifically novel and practically significant results, both experimental and clinical studies were conducted. The experimental part consisted of two series. In the first series, the parameters of acute toxicity of Osteogenon were investigated in intact rats by oral intragastric administration of different doses of the drug; the animals were isolated and deprived of food, while maintaining free access to water, for four hours after administration, after which they were kept under standard conditions. In the second series, the rats were divided into four groups: the control group (n=7) included animals without hepatobiliary pathology that underwent dental implantation without pharmacological support; the experimental group (n=7) included rats with modeled chronic toxic hepatitis that underwent dental implantation without pharmacological support; the comparison group (n=7) included rats with modeled chronic toxic hepatitis that underwent dental implantation and received the hepatoprotective agent Ursokhol at a dose of 80 mg/kg; and the main group (n=7) included rats with modeled toxic hepatitis that underwent dental implantation with pharmacological support consisting of ursodeoxycholic acid at a dose of 80 mg/kg and Osteogenon at a dose of 180 mg/kg. Clinically, 39 patients with partial edentulism of the upper and lower jaws were examined and treated, including 21 patients with partial edentulism of the posterior mandible and 18 patients with partial edentulism of the posterior maxilla; the patients were aged 21 to 55 years, with 20 males and 19 females. The patients were divided into three groups: a control group of 12 patients without concomitant pathology who underwent dental implant placement and received standard treatment; a comparison group of 12 patients with hepatobiliary pathology who underwent implant placement and received standard treatment; and a main group of 15 patients with hepatobiliary pathology who underwent implant placement and received pharmacological support consisting of standard therapy supplemented with Ursokhol 250 mg (one capsule three times daily for two weeks before and two weeks after surgery) and the osteoprotective agent Osteogenon (one tablet twice daily for one month, including two weeks before and two weeks after surgery). The comparative analysis demonstrated that complications after dental implantation in patients without somatic pathology occurred in 16.7% of cases, including 8.3% with wound dehiscence and 8% with postoperative wound suppuration. In the comparison group, complications were registered in 33.3% of patients, including 8.3% with wound dehiscence, 8.3% with postoperative suppuration, 8.3% with peri-implantitis, and 8.3% with implant disintegration. Hepatobiliary pathology was shown to negatively affect implant integration and increase the incidence of complications, with the proportion of patients experiencing complications rising from 16.7% to 33.3%, and the complication rate per placed implant increasing from 5.9% to 19.2%. In the main group, the proportion of patients with complications decreased from 33.3% to 20%, approaching the control group values, while the complication rate per implant decreased from 19.2% to 9.4%. These findings demonstrate a statistically significant positive effect of pharmacological support with hepatoprotective and osteoprotective agents on implant survival and osseointegration in patients with hepatobiliary pathology.
