Essay

The dissertation is devoted to improving the diagnosis, prediction, prevention, and surgical treatment of acute oroantral communications (OACs). These complications most commonly occur after extraction of maxillary posterior teeth, surgical removal of radicular cysts, and dental implant placement. The aim of the study was to improve treatment outcomes by optimizing diagnostic and predictive approaches and developing a novel minimally invasive surgical technique. The study included a literature review, a survey of dentists, CBCT morphometric analysis, retrospective clinical evaluation, and clinical validation of the proposed technique. Major anatomical risk factors were identified, a predictive mathematical model was developed, and a differentiated treatment algorithm was proposed. The new technique is based on multilayer closure using collagen materials, oxidized regenerated cellulose, mucoperiosteal flap mobilization, and tension-free suturing. It is tissue-preserving, minimally invasive, and suitable for outpatient oral surgery. Clinical validation demonstrated reduced healing time, fewer inflammatory complications, improved tissue preservation, and better long-term outcomes compared with conventional methods. The proposed approach can be recommended for routine implementation in oral surgery practice.
