The dissertation work is devoted to elucidating the role of lncRNA gene polymorphisms ANRIL (rs1333049), MALAT1 (rs619586), and HOTAIR (rs4759314) in the pathogenesis of malignant neoplastic diseases of the urinary system. The prospective cohort case-control study involved 342 representatives of the Ukrainian population of the North-Eastern region: the main group consisted of 242 individuals with oncological diseases of the urinary system (101 with clear cell renal cell carcinoma [ccRCC] and 141 with urothelial carcinoma of the bladder [UCB], clinical stage I–II according to TNM) and the control group consisted of 100 individuals without an oncological history. DNA genotyping from venous blood was performed using Real-Time PCR with TaqMan assays on a QuantStudio 5 DX instrument. For the first time, allele and genotype frequencies for the studied loci were determined in the control group: 0.48 for rs1333049, 0.02 for rs619586, and 0.05 for rs4759314, corresponding to European population indices. A statistically significant difference in genotype distribution between oncourological patients and controls was found only for the MALAT1 gene rs619586 polymorphism (P = 0.039). Carrying the minor G-allele of MALAT1 (A/G and G/G genotypes) was associated with a 3.43-fold higher probability of developing urinary tract malignancies compared to A/A homozygotes (P = 0.037, OR = 3.43; 95% CI: 1.07–10.97). The ANRIL gene rs1333049 locus was identified as an independent predictor of ccRCC development, with the minor C-allele associated with a 3.1-fold increase in risk (P = 0.040, OR = 3.10; 95% CI: 1.05–9.18). The HOTAIR gene rs4759314 polymorphism showed no overall association with oncourological risk (P = 0.944). Marked sexual dimorphism was established: in males, the homozygous C/C genotype of ANRIL rs1333049 increased ccRCC risk 3.12-fold (P = 0.05, OR = 3.12; 95% CI: 1.0–9.75); the minor G-allele of MALAT1 rs619586 was associated with ccRCC primarily in men (P = 0.008), whereas in UCB it predominated in women (P = 0.013). Molecular-phenotypic associations revealed that the MALAT1 A/A genotype was linked to greater height and body weight (P < 0.0001) but not BMI; for ANRIL rs1333049, the C-allele exerted a protective effect against UCB in individuals with BMI < 25 kg/m² (P = 0.04, OR = 0.22), which was neutralized at BMI ≥ 25 kg/m²; HOTAIR G-allele carriers demonstrated higher BMI (29.65 vs. 26.72 kg/m², P = 0.042). A phenomenon of genetic effect inversion at ANRIL rs1333049 was identified depending on smoking: in smokers, the C/C genotype was a risk factor for ccRCC (P = 0.040, OR = 3.64), whereas in non-smokers, carrying the C-allele was protective (P = 0.016, OR = 0.17). MALAT1 variability was shown to determine disease aggressiveness: the G-allele was associated with an 11-year earlier manifestation of UCB (65 vs. 76 years, P = 0.032), a 2.74-fold increased risk of metastasis (P = 0.02, OR = 2.74), and significantly reduced overall survival in ccRCC patients (P = 0.0101). Based on these patterns, an algorithm for clinical-genetic stratification of patients was developed to establish personalized monitoring and lifestyle management programs.
