Essay

The dissertation is considering the issue to improvement of the results of surgical treatment of tumours of the spine.The work is based on the analysis of 136 observations of patients with tumours of the spine who were treated in the Institute of Neurosurgery named after academician A.Romodanov Academy of Medical Sciences of Ukraine during 1985 - 1999 years. All patients were subjected to surgical treatment.We have framed the new classification of tumours of the spine what is based on clinical and morphological analysis these tumours. In this work the clinical displays of tumours of the spine are investigated depending on their histological type, axial and transversal localisation, character of diffusion of the tumour. We have estimated of various diagnostic methods in the diagnostic complex necessary for determination of medical tactics at the patients with tumours of the spine. MRI is the best diagnostic methods for definition the indications of localisation of the tumour and indications to surgical treatment. Other methods of tool diagnostics (CT, radiation diagnostic, spondylography, biopsy) were surveyed in addition to the MRI at some categories of the patients. Application of adequate instrumental examinations for patients with tumour of the spine allows to obtain sufficiently full information for defining the medical tactics in these categories of patients, planning of surgical treatment. We have found criteria of the choice of the approach to the tumor, of completeness of performance of operation, way of stabilisation of the spine. Use of the developed complex diagnostic and therapeutic measures allowed considerably improve results of surgical treatment of the patients with tumours of the spine, reduce a postoperative lethality from 14,8 % up to 4,4 %, achieve good and satisfactory results of treatment at 83 % of the patients. The basic results of work have found useful in clinical practice.
