Essay

49 patients with different fractures and fracture-dislocations of proximal humerus. The aim of work - to improve functional outcomes of the treatment of patients with fractures and fracture - dislocations of proximal humerus by the working out and using of new techniques for surgical treatment. The methods of investigation: clinical, roentgenological, MRI-tomography, topograhpo-anatomical, biomechanical, statistical. New knowledge of less traumatic variants of humeral head reduction in the case of anterior fracture-dislocation was obtained at first time. On the base of new knowledge the methods of open reduction of humeral head in the case of anterior fracture-dislocation on the level of anatomic neck or on the level of surgical neck combined with major tubercule fracture were worked out. On the these methods of reduction Ukrainian patents N40880 and N40196 were obtained. The criteria of stable osteosynthesis were based for the cases of proximal humerus fractures and on this base metal construction were worked out (Ukrainian patent N38498 and N38523). Graphanalutical methods revealed that stable osteosynthesis was performed if metal construction crossed fracture line on the angle equal to 90(. The results of investigation were applied in the practical activity of the department of emergency traumatology and reconstructive surgery of Sytenko Institute of Spine and Joints Pathology AMS Ukraine, traumatological department of Meshaninov Kharkov City Hospital of Emergency and traumatological department of Kharkov Region Clinical Traumatological Hospital.
