Essay

The object of research - patients with pathological fractures of long bones extremity with bening and malignant tumoural lesions. The purpose - to improve the results of treatment (prolongation and rise the life quality) of the patients with pathological fractures of long bones extremity with tumoural lesion by working out a tactical treatment and new saving extremity operations. Methods - informatic, clinical, electrophisiological, X-ray, immunological, biochemical, morphological, statistical. The work helped to reveal how biomechanical researches facilitate the process of identifying the character of the tumour (benign or malignant) and the degree of malignancy (the level of the tumour process). As a result of research we elaborated the technique of monopolar endoprosthetic repair and defined the indications for the extremity amputation and the expediency of preoperative inbone injection the cytostatic medicine. Clinical approbation of the given technique in complex treatment of 70 patients with pathological fractures of lengthy bones resulted in considerable improvement of treatment outcomes. For 69,2 % of the patients the good function outcomes are obtained. The operation, which carried spent in good time and by indicathions, allows to prolongate the life of 83,3% of the patients in period of three years and more. It was proved that the saving operation, carried out in case of malignant tumour process, improves the life quality of the patient. The presence of the extremity and its satisfactory function influence positively on the psychological state of such patients. The scientific and practical outcomes are inserted in clinical practice district traumatological hosptal of Donetsk, Scientific resierdg institute of traumatology and orthopedics of Donetsk state medical university named by M.Gorky, orthopedic-traumatological departments central municipal hospital of Vugledar of Donetsk region, district clinical hospital of Zaporogia, municipal clinical emergency hospital №6 of Simferopole.
