Essay

Dissertation is dedicated to peculiarities of adaptative response of hypothalamo-hypophysio-adrenal system (HHAS) to severe brain injury (SBI). The study of 82 patients (18-81 years) was carried out after SBI. All persons showed increase in excretion of glukokorticoids (17-KGS) in each term after SBI, also as discorticism due to lowered production of androgenes (17-KS). The prognostic criteria were proposed for SBI, which are based on pessimistic sense of reliable increase of 17-KGS level and deep decrease of index 17-KS/17-KGS on 3rd -10th days after SBI. Patients who were operated upon with acute intracranial hematomas show earlier normalisation of hormons' levels in comparison to those who were managed without surgery. The using of dexamethasone was proposed in early terms to achieve a rapid restoration of altered HHAS functions and to shorten the stay in neurosurgical department.
