Essay

The objects of research are old injuries of ankle joint complicated by the posttraumatic arthrosis. Objective is the elaboration and ground of improved technology of non-invasive technology of arthroscopic compression arthrodesis of ankle joint for the treatment of patients with posttraumatic arthrosis, which allows to decrease the invasiveness of operation and to improve the results. The methods are clinical, roentgenological, electric physiological, biomechanical and statistic. For the first time it was elaborated and grounded the new method and technology of non-invasive arthroscopic compression arthrodesis of ankle joint for the treatment of patients with posttraumatic arthrosis, that allows to perform the resection of full value of joint surfaces, to provide the stable fixation in the apparatus of external fixation and to make the indications for this operation. The expedience of performing of this kind of surgery step by step including the assembling of compressive-distractive apparatus of external fixation, one stage distraction for the enlarging of ankle joint's cavity, arthroscopic revision of this joint and resection of surfaces and finally one stage compression in the aforesaid apparatus of external fixation. Basing on the comparative analysis of clinical and roentgenological results of arthrodesis of ankle joint, electric physiological and biomechanical evaluation of the results of arthrodesis firstly there was founded the dynamics of restoration of blood circulation, static and kinematical and bearing functions of the operated extremity after opened and non-invasive arthroscopic compressive arthrodesis of the ankle joint. These data testifies about the high efficacy of non-invasive method of arthrodesis of ankle joint. This proposed method of surgical treatment was applied in clinical practice in Dniepropetrovsk state clinical hospital named after I.I. Mechnikov, is using in the study process in Chairs of Traumatology and Orthopedics of Dniepropetrovsk state medical academy, Vinnitsa nationalmedical university, state medical universities of Kharkov and Zaporozhye.
