Essay

The dissertation is devoted to the development of modern surgical treatment hemispheric gliomas with application of modern laser technologies. Work is based on surgical treatment of 205 patients with hemispheric gliomas. Experimental part of the study was performed by tumor modeling in rabbits using the culture of rats experimental glioma (stamm 101.8).Research was carried out with application of high-energy surgical CO2 laser which able to generate the waves of 10,6 microns and Nd: YAG laser generating a waves of 1,06 microns. Preoperative examination of patients includes CT (194 cases), MRI (60 cases), SPECT (52 cases), cerebral angiography (18 cases).We developed and implemented in clinical practice specific algorithm for management of hemispheric gliomas which allows to use the laser technologies differentially and most effectively. We used CO2 laser during removal of hemispheric glioma in 41 cases, Nd:YAG laser - in 138 cases and both of them - in 26. Total tumor removal was achieved in 80 patients (39%), subtotal in 121 patients (59%), partial in 4 patients (2%). Application of laser technologies during removal of hemispheric gliomas provides improved level of surgery which resulting in higher volume of tumor removal and diminishing brain tissue trauma due to the intervention that improve patients life quality.
