Essay

The work deals with the study of pathogenetic role of occlusive-stenotic carotic lesions in developing ischemic strokes and the substantiation of surgical treatment efficiency at an early acute period. Based on generalized results of complex investigation of 124 patients with ischemic stroke, provoked by occlusive-stenotic lesions of carotids, out of whom 94 were operated on and 30 were treated only by medicamentous way, it has been shown that complex treatment including reconstructive surgical interventions has advantages. Surgical correction in occlusion and critical stenosis of internal carotid artery has been confirmed to be more effective at an early acute period versus at an delayed one. It allows improving a patient's outcome after stroke, getting more evident regression of neurological symptoms, improving cerebral hemodynamics. Effectiveness of treating patients with ischemic strokes, caused by occlusive-stenotic lesions of extracranial areas of carotid arteries, in case of an early use of their surgical correction, is considerably higner. After complex treatment (medicamentous with surgical) and rehabilitation 2 years later, working capacity was restored in 85.8% of patients versus 35% of control group patients (non-operated).
