Essay

Object of research - patients with medial fractures of femoral neck. The purpose is to improve the results of pri-mary arthroplasty of proximal part of femur of patients with medial fractures of its neck by the way of differential approach to choice of implant, development of construc-tion of one-pole endoprosthesis and technology of hip re-placement. Methods of research are clinical examination, roentgenological, electrophysiological, biomechanical, statistical processing of information. The domestic endoprosthesis of proximal part of femur was elaborated with participation of author. It was im-proved the technology and instruments arthroplasty of proximal part of femur with participation of author. For the first time the optimal size of stem of endoprosthesis of proximal part of femur was proved on the base of roent-genanthropometric researches. Comparative biomechani-cal research of durability of fastening of Moor, Moor-CITO and ORTEN endoprostheses of proximal part of femur for the first time is carried out; is established, that the ORTEN endoprosthesis design provides the greatest level of reliability of implant fixing, that allows to carry out early and complete loading on operated extremity. The criteria for differential carrying out of realization of arthroplasty of proximal part of femur are developed at medial fracture of femoral neck (MFFN) with integral one-pole implant, one-pole implant with the demountable head and bipolar implant. The application of developed domestic endoprosthesis of proximal part of femur, im-provement of toolkit and technology of realization of operation, of differential tactics of a choice of a one-pole implant kind depending on a concrete clinical situation will allow to improve results of treatment of the patients with medial fractures of femoral neck. The results of work are applied into the clinical practice of Institute of Traumatology and Orthopedic of Academy of medical sciences of Ukraine (Kiev), Kiev medical academy of postgraduate education named after P.L. Shupyc (Kiev), Kiev, Dnipropetrovs'k, Zaporizha, Kharkiv and Cher-kassy regional hospitals, Vinnitsa municipal clinical hos-pital № 2, Sevastopol municipal clinical hospital №9. The materials of work are used in educational process of chairs of orthopedics and trauma of Dnipropetrovs'k state medical academy, Kharkiv state Vin-nitsa national medical university named after N.I. Pirogov and Zaporizha state medical university. One-pole endo-prostheses of hip joint ORTEN are permitted for clinical application by the Ministry of public health Services of Ukraine (Registration certificate №1569 / 2003 from 27.02.2003.) and are applicated into industrial manufac-ture.
