Kozyns'ka I. Clinic and pathogenesis of osteoarthrosis in miners, efficiency of treatment and rehabilitation of the patients with such a disease

The object of the study is coalminers of various professions suffering from osteoarthrosis (OA); the aim is to improve the quality level of OA diagnostics with miners of various professional groups considering mining conditions of mines, to expose new links of pathogenesis of the disease, and to improve the effectiveness of curative and rehabilitation measures; the methods include clinical, radiological, ultrasonic, bio-chemical, radio-immune, immune-ferment, and statistic ones; the novelty is that for the first time peculiarities are determined of clinical and radiological process together with interconnection between periphery OA, spondylarthrosis and osteochondrosis of the backbone with miners belonging to various professional groups, the importance is proved of destructions in lipid peroxidation, antioxidant protection, nitric oxide metabolism, eicosanoids, glucocorticoidal and sexual hormones in the pathogenesis of the disease and development of secondary reactive synovitis, as well as these factors dependence of mining conditions of coalmines, the effectiveness is demonstrated as for complex utilization of non-steroid anti-inflammation medicines, antiaggregants, poly-ferment mixes, chondro-modifiers, acupuncture, electromagnetic field, laser beams, balneological and mud treatment with miners of various professional groups suffering from OA; the results are that criteria are determined for OA course evaluation with miners and the effectiveness of disease therapy forecasting in special professional groups, the rational medical treatment and rehabilitation techniques is developed for such patients; introduction into the practical activity of Rheumatologic Division and Rheumatologic Hospital of Donetsk Regional Clinical Territorial Medical Union, Therapeutic Clinics of Institute of Urgent and Restoration Surgery named after V.K.Gusak of Academy of Medical Sciences of Ukraine, Donetsk Regional Clinical Hospital of Occupational Diseases, Donetsk Municipal Clinical Hospital No.3, Lugansk Regional Clinical Hospital, "Yubileinyy" Sanatorium in the City of Slovyansk, as well as into the pedagogic process of Donetsk State Medical University named after M.Gorky; the sphere of application is curative and diagnostic process at rheumatologic, therapeutic, professional pathology divisions, and academic process of chairs of therapy in medical higher educational institutions.
