Essay

Object of research are patients with bilateral degenerative-dystrophic injury of hips. The purpose – to improve the results of treatment of patient with bilateral degenerative-dystrophic hip involvement by the way of development of the native technology of hip replacement and perfection of methods of preoperative planning and postoperative rehabilitation of patients with this kind of pathology. Methods of research – general clinical, orthopaedic examination, research of the subjective feeling of pain (Mac-Gill Pain Questionnaire, Visual Analogue Scale), roentgenanthropometric data (definition of cortical index according to Barnet-Nordin, Singh index, type of the proximal part of femoral bone acc. to Dorr et al, Spotorno-Romagnoli’s index, index acc. to Noble), ultrasound densitometry “Achilles+” of calcaneus bone, double photon X-ray densitometry LUNAR of the proximal part of femur, computer gait analysis, functional results of treatment acc. to M. D’Aubigne-Postel, Charnley and Harris scores and special score of bilateral hip replacement, statistical processing of information. New knowledge: the method of preoperative planning of bilateral hip replacement with using of special orthopedical slide-rule was elaborated (UA patent 7721); this slide-rule allows to measure the symmetric location of the centers of rotation of endoprostheses. For the first time based on the complex evaluation of clinical and functional state of both hips the criteria of determination of the optimal interval between operations of hip replacement were elaborated and also the indications for the choice of the method of fixation of the implant in patients with bilateral hip involvement. For the first time the dynamic of changing of bone density after hip replacement in patients with bilateral hip involvement was studied and also its connection with the changing of static and dynamic function of lower extremities. The native cementless hip endoprosthesis with press-fit fixation and set of instruments for implantation were elaborated (UA patent 59068 A). This endoprosthesis was also used in cases of bilateral hip replacement. For the first time it was distinguished and grounded the periods of rehabilitation treatment and the postoperative rehabilitation program after bilateral hip replacement was improved. For the first time the scale of evaluation of results of bilateral hip replacement was developed. The materials of work are used in educational process of chairs of orthopedics and trauma of Dniepropetrovsk state medical academy, Kharkov state medical university, Vinnitsa national medical university named after N.I. Pirogov, Zaporozhye state medical university. The results of work are applied into the clinical practice of Institute of pathology of spine and joints of Academy of medical sciences of Ukraine named after prof. M.I. Sitenko (Kharkov), Dniopropetrovsk, Zaporozhye, Kharkov, Cherkassy state hospitals, Vinnitsa municipal clinical hospital N2, Sevastopol municipal hospital N9. Traumatology and orthopedics.
