Essay

Dissertation is devoted to the questions of course of the uncomplicated subarachnoidal hemorrhage of aneurismal genesis for elderly patients. Substantiation and introduction in practice of terms and methods of neurosurgical treatment, according to the atherosclerotic disease of main vessels of head and neck, their deformation, presence of other somatic pathology and features of clinical course of disease and morphological specialty. The morphological part of work proves that for elderly people an arterial vascular spasm which develops as a result of break of arterial aneurysm arises up rarer, and if develops, flows heavier. Upon analyzing the frequency of origin and expressed of basic clinical symptoms it is set for the patients of different age groups, that clinical course of uncomplicated subarachnoidal hemorrhage for elderly people aneurismal genesis has the features that consists of 19,7% frequency and expressed of headache, than for young patients, and 7,2% frequency of origin of convulsive syndrome, on 19% lower frequency of expression of arterial vessels spasm on arriving to the hospital, on 8,8% lower - ischemic complications and much higher frequency of presence of somatic pathology. It is proved that for elderly patients' choice of method of operative interference is grounded on the features of atherosclerotic defeat of arteries and presence of their deformations. On the basis of analysis of frequency of intraoperational and postoperational complications, general results of intracranial operations on shutting of arterial aneurysm from a blood circulation proved that for elderly patients are advised to execute on 13-14th day after a subarachnoidal hemorrhage.
