Object of research: diseases and injuries of musculo skeletal system which are accompanied with the formation of cavity bone defects. The aim: to substantiated experimentally-morphologically, theoretically and clinically the differentiated use of hydroxylapatite ceramic granules varied in porosity and porous biphasic granules for the plasty of cavity bone defects in the region with differing levels of loading. The methods of investigation: clinical, roentgenological, experimental, morphological, morphometric, biomechanical, mathematical (the method of finite elements), statistical, MRI and computer tomography. Scientific novelty: comparative analysis of the formation of bone-ceramic fusion and biomechanical analysis of the strength of bone-ceramic complex, that were generated after implantation of hydroxylapatite and biphasic ceramics with different construction in the regions of skeleton with differing levels of loading were carried out; tension alterations at modeling of cavity defects in the regionsof bone with differing levels of loading were investigated using the method of finite elements, the necessity of differentiated choice of bone substitute depending upon the the range of loading in the site of implantation was proved; an optimal type of calcium-phosphate ceramic for the plasty of cavity defects of acetabulum in the case of its revision arthroplasty was determined using the method of finite elements; different granules of calcium-phosphate ceramic, that were manufactured in the KhNU, were investigated for the first time in the clinical condition; the principals of differentiated use of the different types of calcium-phosphate ceramic for the plasty of cavity defects of bone in the region of skeleton varied in loading with the purpose of providing of required strength in damaged bone section were worked out and put into the clinical practice. The results were introduced in the practice of pediatric orthopedics, emergency traumatology and reconstructive surgery departments of Sytenko Institute of Spine and Joint Surgery AMS of Ukraine, Scientific-Research Institute of Traumatology and Orthopedics of Donetsk State Medical University named after M. Gorki, traumatological department of municipal clinical hospital # 11 of Odessa, traumatological department of municipal clinical hospital # 1 of Poltava; were taught at the department of orthopedic, traumatology and MMM of Odessa State Medical University.
