The thesis is devoted to a relevant problem of treating elderly and senile patients with fractures of the proximal end of femur. The thesis is based on the results of 230 patients including conventional osteosynthesis according to AO/ASIF system in 118 patients, osteosynthesis by blocked implants - in 42 pts, metallocement osteosynthesis - in 70 pts. Solidity characteristics for immobilizing conventional, blocked and metallocement implants to the proximal end of femur have been determined by using biomechanical experimentations. The reaction of the proximal end of femur to the inclusion of implant with polymethylmetaacrylic cement has been studied while experimenting on animals. Research results have proved that taking into account the localization and character of fractures according to AO/ASIF classification as well as the presence and degree of marked local systemic osteoporosis, bone tissue fractures of the proximal end of femur in choosing a technique of osteosynthesis has enabled increasing treatment efficacy, reaching effective distant results in 83,3±6,2 - 85,4±5,1% of cases, satisfactory outcomes - in 14,6±5,1 - 16,7±6,2%.
