Essay

This research is based on results of observing and treating 111 patients whose 156 feet were complicated by hyper- or hypocorrection after myotenoplasty. The given complications have been determined to occur in all age groups and at all stages of disease. The results obtained allow one to state that hypercorrection after myotenoplasty involves not only the position of great toe but also involves 1 metacarpal bone and the position “hammock” of its head, that is, all 3 components of basic mechanism of spread foot are involved. In hypocorrection, similar correlation occurs, but certainly, in contrary format. Using correlative analysis, pre- and intraoperation risk factors for appearing hyper- and hypocorrection as well as ways of immediate prevention of these complications have been developed. The correlation of these ways with the character of spread foot process is illustrated in corresponding schemes of myotenoplastic correction in each from 4 stages of disease. And the developed basic surgical technology was used. The given data testify that despite rather optimistic results of treating patients with hyper- and hypocorrection, certainly, the most perspective direction of optimizing myotenoplasty of spread foot as a selfcompetent operation is prophylaxis of its specific complications.
