Object of research ? the damage of Hill-Sachs of the as a result primary traumatic dislocation of shoulder; purpose - to improve the results of treatment (to prevent the development of the instability of humeral joint, to reduce the period of disablement) in patients with the damage of Hill-Sachs of the way of the development of scientific theoretically substantiated and clinically approved system of complex treatment with the use of the developed method of operational interference and special tool; research methods- clinical, X-ray study, special inspections (ultrasonic study, spiral computer tomography, the magnetic-resonance tomography of humeral joint), mathematical simulation by the method of final elements; scientific novelty - for the first time in the practice of traumatology and orthopedics of the Ukraine it is formed the differential system of diagnostics of the damage of Hill-Sachs in the acute period after the elimination of the primary traumatic dislocation of shoulder. The proposed by theauthor procedure of operational interference it is biomechanically led with the aid of the method of final elements. It is led the advantage of the developed by the author procedure over the existing methods of treating the damage of Hill-Sachs. The algorithm of diagnostics and methods of treatment proposed makes it possible to divide patients with the data by damage on those, whom it is possible to treat by the conservative method, also, on those, which are necessary surgical treatment. Is proven the effectiveness of the procedure proposed, which prevents to the formation so of that called of „like as axe " head of humeral bone, to the development of the instability of humeral joint and subsequently ensures the postoperative restoration of the anatomical and functional parameters of the traumatized joint; the scientific and practical conclusions of dissertation work are inculcated in the training process of the department of medicine of catastrophes and military medicine of the Dnepropetrovsk state medicalacademy MHS of the Ukraine, department of surgical diseases, traumatology and orthopedics of the highest public education institution of the Ukraine „Ukrainian medical stomatological academy " MHS of the Ukraine city Poltava, in the practical activity of the orthopedist-traumatological department of the municipal clinical hospital of № 16 MHS of the Ukraine, Dnepropetrovsk city and the municipal clinical hospital of № 1 MHS of the Ukraine, Poltava city; the field is - traumatology and orthopedics.
