Essay

The dissertation is dedicated to a problem of diagnostics and surgical treatment of medullocervical intramedullar tumors.The researched clinical material is based on 128 observations of patients with medullocervical intramedullar tumors, who were treated in Institute of neurosurgery in the period 1988-2007. Clinical symptoms were systematized in the patients with medullocervical intramedullar tumors depending on their primary locating and diffusion. Microtopographical researches medullocervical intramedullar tumors have allowed to offer differentiated surgical tactics of treatment such tumors which includes finding of the future operative tactics and expediency of carrying out radiotherapy in cases of tumors of different histological structure. Morphological features of this group of tumors are studied, changes in a perifocal zone of a spinal cord in patients with astrocytomas and ependymomas which to structural attributes are similar secondary aksonotomy which develops at patients with a craniocerebral trauma - diffuse axonal injury (DAI) for the first time have been taped. For the first time the estimation of results of surgical treatment of patients with this group of tumours on a scale of an estimation of quality of a life of operated spinal oncologic patients RAND 36-Item Health Survey 1.0 is lead; dependence of results of surgical treatment from histological structure of tumors, its localizations and the sizes is taped.
