Chernyak V. Prosthhetic replacement of the knee joint in patients with rheumatoid arthritis

The thesis is devoted to the actual problem: rise of the treatment efficiency of patients at the late stages of rheumatoid arthritis with lesion of the knee joint on the basis of elaboration of the system of preoperative preparation and improvement of prosthetic replacement technique. On the basis of analysis of treatment of 78 patients with rheumatoid arthritis whom 104 operations of total replacement of the knee joint have been made, there were defined anatomic and functional changes in the knee joint and the injured limb, system of preparation for the knee joint prosthetic replacement has been elaborated that includes elimination of flexion contractures in the knee joint by means of stage-by-stage redressments with application of plaster bandages, and with stable contractures, Volkov-Oganesian device, the technique of prosthetic replacement in cases of rigidity and ankylosis of the knee joint has been improved, the necessity of the use of bone defect autoplastics has been proved. The structural andfunctional state of bone tissue has been studied, and the necessity of screening observation of the bone tissue state and the corresponding medicamentous correction in preoperative and postoperated periods substantiated.
