Simbirtseva K. Clinical and pathogenetic substantiation of calcium metabolism disorder correction of therapy in patients with osteoarthritis and arterial hypertension

Subject of research - osteoarthritis patients with concomitant arterial hypertension; aim - to increase the efficacy of treatment of articular syndrome and arterial hypertension in osteoarthritis patients due to elaboration of adequate methods of correction of calcium metabolism disorders; methods - clinical, ultrasonic, radiological, biochemical, immunoenzyme, statistical; novelty - for the first time it was proved the value of calcium metabolism disorders for progression of bone tissue changes and articular syndrome in osteoarthritis patients with concomitant arterial hypertension, it was established the heterogenic structure of bone tissue in such patients and its dependence on age, calcium metabolism, radiological stage of osteoarthritis and arthrologic status, it was specified the role of nitric oxide deficiency in the occurrence of calcium metabolism disorders and bone tissue changes, it was extended the conception of indapamide influence on calcium homeostasis and its value for antihypertensiveand articular effects, it was proved the appropriateness of correction of calcium metabolism disorders and endothelial nitric oxide deficiency for increasing the efficacy of treatment of such category patients; results - it was shown that correction of calcium metabolism disorders and endothelial nitric oxide synthesis in osteoarthritis patients with concomitant arterial hypertension based on clinical, laboratory and instrumental studies decreased bone tissue remodeling, improved cartilage function, optimized 24-hour blood pressure profile and increased the efficacy of antihypertensive therapy; introduced in practice of Donetsk Railway clinical hospital, hospital with polyclinic of Ministry of Internal Affairs of Ukraine in Donetsk region, therapeutic clinics of Institute of urgent and restorative surgery named by V.K.Gusak, Academy of Medical Science of Ukraine, medicosanitary unit №12 (Kharkiv), regional clinical hospital named by M.I.Pyrogov (Vinnytsya), municipal clinical hospital №1 (Lugansk), municipalhospital №5 (Kryvyi Rih), in educational process of Donetsk national medical university named by M.Gorky, Vinnytsya national medical university named by M.I.Pyrogov, Dnepropetrovsk state medical academy, Kharkiv national medical university and Lugansk state medical university; area of using - treatment-and-diagnostic process of rheumatologic, therapeutic departments, educational process in therapeutic institute of higher education.
