Essay

Diagnostic basic of extramedullary tumours of CCJ is the establishment of features of clinical current of disease and well-timed application modern neurovisual research methods. The adequate choice of a surgical approach depends on the localization of the tumor, its size and the extent to which it has spread. Far-lateral and posterior-lateral approaches in most cases are the most optimum and the least traumatic in cases of extramedullary ventrolateral tumors of craniovertebral and upper cervical localization.The extreme lateral approach is advisable in cases of big size ventral craniovertebral tumors. The factors, increasing the risk of unsatisfactory functional outcome of a surgical intervention are dense consistency of the tumor, its big size and tumor tightly connection with vital neurovascular structures. Application of modern radiology techniques and the developed operational interventions in view of anatomic features of nervous structures and application of microsurgery in craniovertebral tumor surgery have allowed to reduce a mortality from 37,8% up to 6.25%, and to improve quality of a life of patients twice
