Essay

The survey is based on investigation and treatment of 164 patients with hand involvement in psoriatic arthritis. In 86 % of patients skin lesions preceded to the locomotor system disease. The patients had mostly polyarticular disease (87,3 %). Clinical pattern of hand joints involvement depends on the stage and the form of the disease, and on the degree of pathological process activity. We revealed significant immunological changes and violations of mineral and organic bone base metabolism. Early and late x-ray symptoms were defined. Thermography and ultrasonography, which allowed to evaluate the effect of conservative treatment and to define indications to the operative treatment, were investigations of the great value in the early stages of hand joints involvement. Conservative treatment included drugs and physical therapy. The developed system of reconstructive treatment provided the sequence and continuity on the different steps of medical care to the patients with hand joints involvement. The proposed algorhythm of diagnosis and treatment allowed improving of treatment results.
