Petrov V. Medical rehabilitation and prosthetic and orthotic management of children with stumps of lower extremities

Object: medical rehabilitation and prosthetic and orthotic management of children with stumps of lower extremities. The purpose: medical rehabilitation and prosthetic and orthotic management of children with stumps of lower extremities. Methods: clinical, radiological, biomechanical, statistical. The concept of rehabilitation of children with stumps of lower extremities has been scientifically justified, the system of rendering of prosthetic and orthotic aid and medical rehabilitation to children - invalids with stumps of lower extremities has been improved. The structure of diseases and defects of stumps has been specified, classification of diseases and defects of stumps of lower extremities in view of stump growth has been improved, the technique of treatment of patients and rational prosthetic and orthotic management has been developed, the standard program of rehabilitation of children with postamputation and congenital stumps of lower extremities, algorithm and periods of realization of this program have been justified and developed. For the first time in Ukraine the periods for change of lower extremity prostheses in children of different age groups have been scientifically justified. Principles of training to use lower extremity prostheses have been developed and criteria of a choice of their designs for children in view of age of the child, a level of absence of the lower extremity have been specified. The results of the work allow to use complex rehabilitational measures for children having absence of a lower extremity owing to amputation or the congenital deficiency. The designed standard program of rehabilitation enables to apply individual approach to the patient when defining medical measures and the design of a prosthetic and orthotic product. The suggested algorithm and periods of realization of the rehabilitational program for children with stumps of lower extremities can be used as a basis for development of reports on medical rehabilitation and prosthetic and orthotic management and can be applied in the work of children's orthopedic departments of city hospitals, regional hospitals, clinics of research institutes and medical departments of prosthetic and orthotic enterprises. The knowledge of the reasons of origin of diseases and defects of the lower extremity stump enable to prevent their origin, and if present to carry out effective treatment and rational prosthetic management. The defined periods of change of prostheses in children enable to lower the number of diseases of the stump, the suggested surgical intervention at different deficiencies of the stump allow to carry out rational prosthetic management with designs of prostheses which ensure maximal functionality of the child.
