Belonoszhkin G. Osteosynthesis of para-intraarticulare fractures of bones by meeting-compressing screws (clinical - experemental research)

The dissertation is devoted for actual problem of treatment patients with para-intraarticular fractures of bones. The results of 328 patients have been fixed in basis of dissertation, including traditional osteosynthesis according to system AО/ASIF for 256 patients, osteosynthesis of para-intraarticular fractures of bones by meeting-compressing screws - for 72 patients. With a help of a biomechanical tests, descriptions of fixing durability of spongiosis screws AO system and meeting-compressing screws to epimethafises of humeral, thigh and tibia bones, have been determined. With a help of mathematical research, using formula, which uses in technique estimation of technological qualities of screw-thread connections, the mechanism of interaction of "bone-fragment-screw" system, compressing effect of cancellous screw of AO system and meeting-compressing screws, have been studied. The results of the work have proved, osteosynthesis by meeting-compressing screws, plates with meeting-compressing and cortical screws, allowed to promote efficiency of treatment, to attain good results in 72 + 5 % of cases, satisfactory results -25 + 5,3 %, unsatisfactory - 2,9 +2,0 % for the patients with para-intraarticulare fractures
