The object of the research is patients, suffering from the ankylosing spondylarthritis (AS), the purpose is to define meaningfulness of violations of central and peripheral mechanisms of hemodynamics, expose and estimate character of the structural-functional state of the heart-vascular system, endothelia and adaptation possibilities of patients with a AS, to ground scientifically the new going to the pharmacological correction of the exposed violations, the methods are generalyclinical, biochemical, immunological, immunofermental, physical and chemical, instrumental, statistical, the novelty and the results - first on the basis of complex research of patients with a AS basic changes are exposed of the morfofunctional state of heart and vessels, which are represented in growth of hypertension in the small circle of circulation of blood, overload of right departments of heart and decline of elasticity and resiliency of vessels, are conditioned by the increase of activity of inflammatory process (in particular, by a cytokinemia), duration of disease, especially for dependency upon steroid hormones patients, connection of pulmonary hypertension with the level of violation of the function of the external breathing and bronchial ability to travel cross-country, the variants of redesign of the left chambers of heart, signs of violations of diastole function of the left ventricle, are selected, possibility of application is scientifically grounded jointly with preparations of base therapy of selective blockers of the calcium ductings of lercanidipin, that positively influences on an endocardiac and pulmonary hemodynamics, it is inculcated in practice of the therapeutic and cardiologic departments of state establishment the "Travelling clinical hospital № 2 stations Kiev" of a transport of a particular branch association the "South-west railway", city hospitals № 2 and № 9 (cardiologic and rheumatological departments) of the Crivoy Rog, in the educational process of department of therapy and domestic medicine of faculty of postdiplomal education of the Dnepropetrovsk state medical academy, the area of the use is a treatment-diagnostic process of the rheumatological and therapeutic departments, educational process of departments of therapeutic type of medical higher educational establishments.
