Essay

In the dissertation the theoretical generalization and a new approach to endoscopic microdiscectomy using that lets to optimize surgical treatment of patients with discogenic cervical spine neurocompressive syndromes are given. The research is focused on improvement of treatment results of discogenic cervical spine neurocompressive syndromes using minimally invasive methods of modern neurosurgery, including endoscopic microdiscectomy. The research is based on the results of clinical papers study and prolonged supervision of 92 patients with cervical spine neurocompressive syndromes, been operated in the clinic of laser-endoscopic spinal neurosurgery of the SI "Institute of neurosurgery named after acad. A.P. Romodanov of Academy of Medical Sciences of Ukraine" in the period from 2001 to 2005 years. 84.8% patients were operated because of discogenic radicular syndrome, 5.4% - because of myelopathy, 7.6% - because of myeloradiculopathy, 2.2% - because of reflectory irritative (angiospastic) syndrome of vertebral artery. On the base of early and remote results analysis of endoscopic microdiscectomy, used at cervical spine intervertebral disks hernias, criteria for patients' selection for surgical treatment were determined, taking into consideration clinical signs and neuroimaging data. High (in 93.4% cases) efficiency and minimal invasiveness of this method at cervical spine neurocompressive syndromes were confirmed. The main results of the research have been provided into neurosurgical practice.
