Pshenychnyy T. Diagnosis and complex treatment of juvenile rheumatoid arthritis

The dissertation is devoted to actual prolem: improving the performance of diagnosis and treatment of JRA based on developing early detection and treatment. Statistically proved that the definition of anti-MCV in patients with JRA has important diagnostic value, because it does not depend on the length, quantity of inflamed joints, stage and phase of disease activity (р<0,007), and is dominated the traditional RF diagnostic value (р=0,46).The characteristic changes in MRI of the knee joint in patients with JRAwere studied and dependence with anti-MCV-AT in patients with different disease duration was found. The effectiveness of arthroscopic surgery versus open synovektomy of the knee joint was proved (p<0.0001). The effectiveness of the knee and hip total arthroplasty over the biological arthroplasty in closer (?2=7,04, p=0,029) and remote periods (?2= 9,5, p=0,008) was proved. Methods of clinical, radiological, biochemical, medical and statistical analysis of the 147 treated patients results demonstrated high efficiency of the proposed integrated approach to treatment (р=0,022). Diagnosis, treatment algorithm and rehabilitation scheme of patients with juvenile rheumatoid arthritis were Developed and clinically tested.
