Essay

Object:: reparative osteogenesis of rat’s thighbone and reparative osteogenesis in cases of long bone fractures of patients in conditions of insertion into defect of the native marrow. Purpose: improving the results of patients’ treatment with diaphyseal fractures of limbs bones by the usage of osteosynthesis by stimulation of reparative osteogenesis due to autoplasty of native marrow. Methods: complex clinical examination, laboratory examinations, radiography and computer tomography of long bones, experimental, methods of histology (with electronic microscopy), statistic. For the first time there is introduced a morphological analysis of the course of reparative osteogenesis after the inserting of cultivated mesenkhymal marrow cells and native marrow into the bone defect of the rat’s thighbone, which allowed to obtain new knowledge about the process of regeneration and to identify features of stagedly-temporal characteristics of reparative osteogenesis of long bones’ fractures, there is developed a pathogenetically grounded, comprehensive approach as to patients’ treating with diaphyseal limb bones fractures and with various kinds of ungrowns. There is developed and instilled into the practice of health care a method of patients’ treatment with long bones fractures based on metaloosteosyntesis and autotransplantation of native marrow. There is developed a device for marrow collection (patent № 60236) and an apparatus of external fixation for the reposition of long bones’ fragments and for compressive-distractional osteosynthesis (patent № 49002). The Results of the investigation are instilled into practical activities of the Traumatology and Orthopedics offices of Transcarpathian Regional Clinical Hospital, Uzhgorod City Hospital, Khust, Vynohradiv, Tyachiv District Hospitals, Zaporizhzhya Regional Clinical Hospital, Kyiv Regional Clinical Hospital № 1.
