Essay

Object: regeneration of bone tissue in conditions of usage of dehydrated bone bioimplants in the treatment of benign tumors and tumor-like bones in children. Aim: improvement of treatment results of tumors and tumor-like lesions of bones in children by justification of possibilities of usage of dehydrated bone bioimplants based on experimental investigations, development of rational methods of bone plasty. Methods: clinical, roentgenologic investigations, magneto-resonance computer tomography, morphometric, biomechanical, experimental modeling; histologic, statistic methods. For the first time impact of de- and rehydration on the process of remodeling of bone bioimplants Tutoplast (cortical, spongy, cortical-spongy) was revealed. It was established that using bone implants Tutoplast in dehydratated form, processes of bone regeneration occur more actively. Processes of resorption in dehydrated bioimplants was revealed to prevail. It was determined that dehydrated bone bioimplants Tutoplast swell after their implantation and are firmly fixed in the defect at the expense of sorption of tissue fluid. For the first time in the experiment on laboratory animals it was proved that filling of bone defects with dehydrated bone bioimplants Tutoplast in any combination does not decrease strength of bone block as compared to healthy bone. It was established that preceding rehydration of cortical bioimplants Tutoplast in physiological solution 0,9% NaCl leads to decrease of bone block strength, while usage of previously rehydrated spongy component does not lead to negative consequences. Methods of replacement of defect of long bone are proposed, therewith there is no necessity in additional fixation with metal-constructions; it is possibly to use external plaster immobilization only; means of measuring extent of bone portion subjected to resection, favored adequate defining of Tutoplast volume for the further filling of bone defect and its rational usage; device for the processing of a long bone allows to avoid fractures of operated bone segments during opening of tumor cavity and to improve results of treatment of children with benign tumors and tumor-like bone lesions. For the first time basing on clinical material (90 cases under observation) a positive impact of bone bioimplants Tutoplast (cortical, spongy, cortical-spongy) in dehydrated form was proved. Research results were introduced into clinical practice of CI "Dnipropetrovsk regional clinical children hospital", Regional children clinical hospital of Kherson, CI "City children clinical hospital" of Lvov, Children city clinical hospital, Poltava, CI "City clinical hospital N8" of Krivoy Rog, Regional children clinical hospital of Kirovograd.
