Zones-sources of fibrogenesis with populations of fibrogenic а-SMA+ cells were determined on the base of complex analysis of liver biopsies of 120 patients with chronic HCV-infection and analysis of clinical-laborathory data of 268 patients with hepatitis C virus (HCV). The important role of а-SMA+ activated fibrogenic cells' proliferation and apoptosis in liver fibrosis and cirrhosis morphogenesis was established. A compound of immunocellular infiltration in liver was established and foci of immunocellular liver destruction were differentiated due to IHC analysis. The main morphological types of liver fibrosis were determined. Features of perisinusoidal-cenrtolobular liver fibrosis at patients with chronic HCV-infection were also studied. Prognostic signs of possible progression of each morphological types of liver fibrosis were studied and defined. The scoring system was proposed for perisinusoidal-cenrtolobular and mixed liver fibrosis. There were determined features of MMP-9 and TIMP-1 expression at patients with chronic HCV-infection. Complex criteria of stable and active liver cirrhosis were establised. The main pathways of liver cirrhosis progression and decompensation were detaily characterized. Method of immunochromatographic express diagnostics of HCV-antibodies in liver tissue and postmortem blood was designed, patented and embeded in practice.
