Essay

Object: degenerative spondylolisthesis. Aim: to improve diagnostics and enhance the results of treatment of patients with degenerative spondylolisthesis on the base of the study of structural and functional changes, which appeared under condition of such pathology. Methods: clinical, X-ray, roentgenometry, roentgen-contrast, computed tomography, MRI, biochemical, statistical. For the first time: the relationship between main parameters of sagittal vertebropelvic balance and severity of dysplasia and degeneration value of spinal motion segment was determined; features of bone remodeling in the spinal motion segment by degenerative spondylolisthesis were shown; mechanisms of stenosis formation in the spinal canal and mechanisms of dynamic stenosis by degenerative spondylolisthesis were exposed; mechanisms of pain appearance with degenerative spondylolisthesis were found out; hormonal status (triiodthyronine, thyroxine, thyrotropic hormone, somatotropic hormone, estradiole, testosterone) and it relationship with main parameters of sagittal vertebropelvic balance in patients with degenerative spondylolisthesis were detected; methods of conservative and surgical treatment were improved. The research results served formation of new correct conceptions about structural and functional changes which appeared by degenerative spondylolisthesis, that allowed improve and increase quality of diagnostics. Proposed techniques of conservative and surgical treatment of patients with degenerative spondylolisthesis allowed improve and refine results of treatment and quality of life of patients with degenerative spondylolisthesis. The results of dissertation research were introduced in clinical practice of communal institutions "Municipal clinical hospital № 11", SI "Sytenko Institute of Spine and Joint Pathology National academy of medical science of Ukraine", education process in department of traumatology and orthopaedics of Odessa national medical university Одесского национального медицинского университета Ministry of Public Health of Ukraine.
