Knyazeva A. Ways of reducing the cardiovascular risk in subclinical hypothyroid dysfunction in patients with rheumatoid arthritis

Research object - changes in the level of cardiovascular (CV) risk in patients with rheumatoid arthritis (RA) with subclinical hypothyroid dysfunction (SGTD) and under the influence of treatment, target - establish the CV risk in patients with RA and SGTD and optimize diagnostic and therapeutic measures, methods - clinical, biochemical, immunological, enzyme immunoassay, instrumental, statistical, and the novelty of the results - first investigated the level of CV risk in patients with RA with SGTD, determined that patients with RA and SGTD level of CV risk is much greater than in patients without SGTD, the level of the CV risk depends on the degree of activity of the pathological process, the level of thyroid stimulating hormone (TSH) and thyroid peroxidase antibodies (aTPO), worsening of inflammatory activity (IMA), articular syndrome, expressive functional and radiographic changes in the joints and a higher rate of systemic manifestations of RA patients with SGTD, a direct relationship between aTPO and TSH and phase IMA, as well as between the duration of RA and levels of TSH, the progression of endothelial dysfunction (ED) and the worsening of RA in patients with RA and SGTD, the relationship at the level of violations of the intima-media thickness (IMT) and indicators antioxidant capacity, markers of rheumatoid inflammation SGTD, DE and levels of CV risk for the first time pathogenetically expedience and prospects of application of sodium selenite in treatment of patients with RA and SGTD and proved its effectiveness in comparison with baseline therapy; introduced - in practice hospitals - medical department of Zaporozhye KU "Multidisciplinary Clinical Hospital № 9", rheumatology department of Lugansk Regional Hospital, Division of Cardiology, functional diagnosis and therapy NNLK "University Hospital" Donetsk National Medical University of М.
