3. Object: metastatic lesions of long bones. Objective: to justify, develop and implement in clinical practice differentiated approach to the selection of indications and methods of organ surgical treatment of patients with metastatic lesions of long bones. Methods: Clinical , radiation (X-ray and CT scan, MRI, ultrasound), osteostsyntihrafiya , pathomorphological , mathematical modeling, statistical. For the first time defined the survival prognosis of patients with metastases in long bones. It is proved that the main factors that account for when selecting indications for surgery in general and its species is the prediction of patient survival and physical status classification ASA. For the first time as a result of the study of the stress- strain states in the " implant - bone " to fill defects metadiaphyseal models femur reasonable indications for different methods of substitution postresection defects of long bones during surgery in patients with bone metastases. The basic principles of choosing the optimal treatment strategy and methods of surgical treatment of patients with stage metastatic lesions of long bones. Developed and proposed for clinical implementation of lock ( patent of Ukraine № 68657 and № 67040 ) and the new method of substitution postresection defects of the distal femur metaepiphysis allow you to replace postresection defect in the lower third of the femur , to ensure reliable fixation of bone fragments , shorten operation , keep the knee joint and restore the function of the knee joint and the lower extremity in a short time and can be used in any medical facility or hospital where the surgery is carried out metastatic lesions of long bones . The mathematical model allows us to study the stress- strain state in the "implant - femur" and can be used for further research. Developed a computer program to determine the prognosis and survival of patients, respectively , indications for surgery and greatly facilitates the work of a doctor. Principles of selection of the optimal treatment strategy and methods of surgical treatment of patients with metastatic lesions of long bones allow individual approach and apply the appropriate treatment strategy of patients, improve surgical care and reduce the rate of postoperative complications, and this, in turn, allows to reduce the length of stay a hospital and the number of primary disability.
