Essay

Object: tuberculosis оf bones and joints. The purpose is improving the diagnosis of the tuberculosis of bones and joints on the basis of using modern methods of examination and study in experiment and clinic modern peculiarities of clinical and pathomorphological course and establish the diagnostic value of polymerase chain reaction for this disease. Methods: clinical, radiologic, pathological, bacteriological, molecular studies, and statistics. The new experimental model of tuberculosis оf bones and joints was created, which allows to research all stages of the disease (patent of Ukraine № 70401). New knowledges about modern clinical, radiological, pathological features of the course of tuberculosis оf bones and joints were received in experiment and clinic. For the first time in the experiment the delay of radiological from clinical manifestations of the disease were founded at early stages of disease. Based on the design of experimental tuberculosis of the knee stages of course gonitis in the Guinea pig were followed up, which are consistent with the generally accepted classifications. The identity of tuberculosis models of the knee joint in the Guinea pig while infecting to the metaepiphisis of the bone to the main clinical forms of tuberculosis of the knee in human were detected. For the first time it was calculated the total value of the test polymerase chain reaction in the diagnosis of tuberculosis оf bones and joints and it is 88.2 %. Also the main advantage of the polymerase chain reaction are high speed, low cost, security research. Fundamentally important characteristics of the specified method of the examination is its high specificity and sensitivity. The health care system received a new highly effective method of diagnosis of osteoarticular tuberculosis, which allows to diagnose this heavy disease at an early stage in time and thereby enhance the results of treatment. The use of polymerase chain reaction would allow monitoring of the effectiveness of antibiotic therapy. Developed experimental model of the tuberculosis оf bones and joints can be applied in further research activities tuberculous lesions of the joints at different stages of the disease.
