Essay

The dissertation is devoted to evaluation of main clinical and subclinical manifestations of cardiovascular lesions in patients with antiphospholipid syndrome, clarifying the pathogenetic mechanisms of formation of atherosclerotic vascular lesions involving both traditional and specific risk factors and their association with course of disease and state of cardiovascular system. It was analyzed the prevalence of different classes of antiphospholipid antibodies in the Ukrainian population of patients with APS and established the differences in lipid metabolism depending on the type of APS. It was shown that the development of structural and functional changes were closely associated with laboratory markers of endothelial dysfunction - sVCAM and L-selectin, indicators of inflammation - CRP, TNF-? and proinflammatory cytokines, antibody activity, lipid metabolism and decreased of serum paraoxanaze activity. The stratification of total individual cardiovascular risk in patients with APS was provided, during which it was revealed that the risk of cardiovascular events by SCORE did not satisfactorily reflect the real state of the cardiovascular system, evaluated instrumentally.
