Essay

Object: degenerative-dystrophic diseases of the hip joint, femoral neck fractures and their consequences. The goal: investigation of the characteristics. Methods: clinic-anamnestic, radiographic, biomechanical, experimental, electrophysiological, statistical. For the first time on the basis of comparative evaluation of the stress-strain state of various brands of bone cement based on polymethylmetacrilate set variability of the strength properties of cement with time elapsed after polymerization. It was found that the best performance is cement grade Cemex Rx, the strength of which increases monotonically during the 90-days from the date of a polymerization (from 96,6 ± 1,35 MPa to 107,4 ± 1,10 MPa; p <0.05) ; for bone cements brands Osteobond ™ Copolymer Bone Cement by Zimmer, De Puy CMW 1, Simplex Stryker P is characterized by the decline in strength immediately after polymerization and stabilization of cement Osteobond Zimmer and De Puy CMW 1 in terms of 14-60 days, cement Simplex Stryker P - after 60-90 days. First held X-ray morphometric evaluation of the characteristics of the implant, confirming the advantages of the improved design of the acetabular component of a total hip prosthesis for cement fixation type over conventional implants: a uniform distribution of the cement mantle, more subtle and complete filling of cement medullary canal of the femur (p <0,05 ), which provides a strong primary fixation and operational reliability of the endoprosthesis. For the first time through a comparative analysis proved the effectiveness of early (within the first 2-3 days after total hip arthroplasty cement) full load on the operated limb that will ensure the positive functional results in 93.9% of cases, two months after the operation. The results of an experimental study of strength and stiffness characteristics of different types of bone cement into the dynamics were the basis of the recommendations on the timing of the full load on the operated limb after surgery: cement Cemex Rx - immediately after surgery; for cement brands Osteobond Zimmer, De Puy CMW 1 - after one month; cement Simplex Stryker P - at the expiration of 1,5-2 months. Developed and implemented in practice acetabular component hip endoprosthesis cement fixation type (Ukrainian patent number 90795 for utility model "acetabular hip prosthesis"), contributing to the uniform distribution of the cement mantle, thus increasing operational reliability, primary fixation and start early load on the operated limb The results of the thesis put into practice CE "Dnipropetrovsk Regional Clinical Hospital named after I.I.
