Essay

Object: the rehabilitation of patients with lack of function for self-dependent walk due to paresis and paralysis. Aim: improving the efficiency of formation and recovery of function of walk in children with spastic and flaccid palsies and paresis by the grounding and application of the orthosis with reciprocal mechanism. Methods: clinical, radiological, functional, statistical. For the first time scientifically proved expediency of application of the orthosis with a reciprocal mechanism and its place in the system of rehabilitation of children with severe disturbances of a locomotor system function due to spastic and flaccid palsies and paresis. Indications to ensure the orthosis with a reciprocal mechanism of children with spastic and flaccid paralysis have been clarified. First the method of rehabilitation of children with a lack of function for self-dependent walk due to spastic and flaccid palsies and paresis with the use of the orthosis with a reciprocal mechanism (patent 93074 UA) was developed. For the first time the system of rehabilitation of children with absence of stepping movements with the release of a particular stage of a walk in the reciprocal orthosis was founded and developed. First the effectiveness of the developed rehabilitation system for the formation and the recovery walking function of patients with spastic and flaccid palsies and paresis was evaluated. Influence of a motor mode in the orthopedic orthoses on the status, the functional state of muscular, cardiovascular and pulmonary systems were analyzed. The proposed conceptual model of the recovery of the vertical posture and walking can be used in further scientific research concerning a rehabilitation of patients with lack walking function due to other diseases of the musculoskeletal system. Application of the reciprocal orthosis will allow to reduce time to recover (or forming) function independent walk in children with spastic and flaccid paresis and palsies. Reasonable indications for use of the reciprocal orthosis help doctors (pediatric orthopedics and neurology) timely appoint them for recover (or forming) function of movement of patients with spastic and flaccid paresis and palsies. The proposed methodology for the complex rehabilitation will improve the efficiency of the formation and recovery of walking function and general state of children with spastic and flaccid paresis and palsies. Results of the study were introduced into the clinical practice of the Ukrainian Research Institute of Prosthetics, Prosthesis and Restoration of Working Capacity of Ministry of Social Policy of Ukraine and public health agencies "Regional baby house № 3" Kharkiv Regional State Administration.
