Danevych О. Diagnostic and surgical treatment for ACTH-secreting pituitary adenomas

The thesis is dedicated to the diagnosis and surgical treatment of ACTH-secreting PA, by improving diagnostic algorithm for СD, refining topographical features of tumors, optimization techniques of endoscopic surgery and microsurgery and determining the basic criteria of surgical treatment. Based on the study diagnostic system for CD was improved. The basic indications for surgical treatment of ACTH-secreting PA were determined: identifying clinical features of CD or NS, increased ACTH blood levels and visualization PA by MRI. We improved the technique of microsurgical and endoscopic interventions by clarifying microPA topographical variations and determining criteria of radical removal for microPA and macroPA. The basic performance criteria for surgical treatment were defined, and further tactics in case of their failure in the immediate post-surgery period were determined.
