Essay

48 patients with aseptic necrosis of the femoral head (ANFH). The purpose was to develop and introduce into practice different kinds of endoprostheses of the hip joint for primary differential arthroplasty in ANFH on the basis of the study of biomechanical properties of the femoral head. The following methods were used: clinical, X-ray, biomechanical, statistical. Clear notions about the role of elements of the femoral head, particularly the subchondral layer, in the mechanism of load transmission from the contacting surface to the spongy tissue were formed. Use of bipolar and total endoprostheses depending upon a stage in the course of ANFH was substantiated. Hip joint endoprostheses and instruments for their implantation were developed. Good results were achieved in 95.8 %, and satisfactory ones in 4.2 %. This technique was introduced into clinical practice of the Kharkov Research Institute of Orthopaedics and Traumatology, Sevastopol City Hospital No.6, Dnepropetrovsk and Cherkassy Regional Hospita ls.
