Essay

The object of research - 1063 patients with sacroiliac joints (SIJ) injuries and diseases. The purpose - elaboration, experimentally-mathematical basing and clinical verification of pelvis stability conception, forms of it's loss and reneval system attached to SIJ injuries and diseases. Methods of research: mathematical and conceptual modeling , clinical, radiological, MRI, biomechanical, morphometrical, statistical. The new direction in studying the pathomechanics of the pelvic instability in SIJ injuries and diseases has been worked up from the point of the systematic approach. For an establishment of the role of SIJ ligaments in preservation of pelvic stability an original method of experimental studying and a device for its realization were elaborated. The data received in the experiment made it possible to develop a mathematical model of pelvis and a conception of pelvic instability. Offered adapted pelvis model, created with use of finite elements method is based on computer representations processing, got attached by MRI. On the basis of pelvic instability conception according to the data of experimental, clinical, roentgenologic and MRI investigations the algorithmus of diagnosis and treatment for sacroiliac leisures has been developed. The tactic of treatment for SIJ injuries is determined by a form of pelvic instability. The use of an adapted mathematical model of the pelvis for differential approach to pelvic stability restoration made it possible to substantiate the system of radical - preserving and reconstructive operations in the treatment of SIJ diseases. Results are used in Sytenko Institute of Spine and Joint pathology of Academy of Medical Scienses of the Ukraine, Мeshchaninov clinical hospital for first-aid and urgent medicine of Kharkov, 17 - th clinical hospital and Regional clinical hospital for traumatology of Kharkov, 9 - th clinical hospital of Sebastopol.
